From healthcare to education -- or is it all the same?

I'm back.

After a 4-year sabbatical in healthcare services (or more specifically health insurance), the passion of applying emerging technologies to societal needs has brought me back to education and workforce development, a field that I had had the pleasure to be engaged in many times before.

However, the excursion to healthcare was no waste of time, as challenging as it might have been. It unearthed many interesting parallels and correlations. My new colleagues must be sick of me constantly describing those parallels in order to create the permission space to apply previous experiences to current challenges. So, here some of those observations -- in no particular order -- that lead me to believe that it is the educational system that needs to be fixed first before we can hope to successfully tackle other societal challenges like healthcare, poverty, or multiculturalism.

Both, healthcare and education are very complex systems of systems. It is these ecosystems that make organic change hard, since the involved stakeholders have found mutually beneficial business models that allow them to co-exist. Changing those business models requires a significant amount of innovation -- and time, which might represent too much of a risk for existing stakeholders to expose themselves to.

Both ecosystems address a fundamental and recognized human needs. However, historically health and education were privileges of the wealthy and the complexity of our ecosystems has prevented them from catching up with the modern realization of the right for healthcare and education. Consequently, while everybody is talking about those rights, nobody is willing to pay or be accountable for it.

Both ecosystems are collecting significant amounts of data, which theoretically could be used to mine valuable insights towards improving the efficacy of the systems, empowering the individuals they are serving and developing new business models to change the fabric of those ecosystems from the ground up. However, the IT infrastructure of both industries is archaic, focused on transaction management, billing and compliance. Major efforts are underway to integrate data from various historical data sources and create more transparency around newly created data, so-called BigData initiatives. However, the end-user individuals that could actually benefit from such data and create contributions to generating insights by injecting their unique experiences, have never been educated or trained in analyzing, correlating, mashing and communicating data in an exploratory and audience-centered fashion. So there is an additional need for significant data literacy that first has to find its way into the educational system -- K-12 and post-secondary -- before any of those data and analytics democratization efforts can be successfully implemented.

From a human perspective, there is much work that needs to be undertaken in the area of behavioral economics in order to get to the roots of why individuals behave in a certain way -- and thus how we can motivate them (and then remind them) to take proper action. We all know that it would be better to eat the broccoli and carrots, but when the tray of cookies is standing in the office, why can't we resist? We all know that we should plan ahead and not procrastinate, make our decisions based on information not hearsay, and choose a career that matches our skills and interest as well as economic requirements rather than what seems trendy and safe. So why do we get it all wrong most of the time?

Once individuals' motivation and triggers are identified, there is the opportunity to create much more effective and successful product and service offerings, and much more compelling and seductive user experiences through user-centered design (UCD). However, both industries -- while heavily employing the term UX -- are driven by the misconception that their subject-matter experts "know what the customer wants," consequently reducing UCD to focus group consultations and justifying design decisions with limited anecdotal evidence. More often than not, even that 'customer' is merely another stakeholder in the system -- often a payor -- and not the ultimate consumer of the product and service. It is, therefore, no suprise that despite decades of published research in user-centered design, usability and human factors, the majority of products and services involving basic human needs are still designed towards optimizing transaction management rather than user experience.

Finally, both industries start realizing that taking a myopic view onto their inherent challenges is insufficient in order to arrive at sustainable and scalable solutions, and that a more holistic view of people's well-being should be used as guidance to develop solutions and assess their success. In healthcare as well as education, the financial economics dictate much of the potential success, whether it is based on the quality of available specialists, schools, or colleges to attend. At the same time, we are painfully aware of how school and job stress can have significant emotional and physical health implications, and vice versa, how poor health inhibits school and career performance. We further realize, how liberating in any of those systems it is to share own experiences with people in the same situation, and how rewarding it is to apply those experiences to help or advise others in those situations, whether it is through mentoring, tutoring, stewardship or care. So, there might even be interesting social business models out there where one aspect of one's well-being subsidizes addressing the needs of others. However, in the absence of such business models, both ecosystems -- healthcare and education -- are struggling in considering a more  holistic view on people's life satisfaction.

What type of social business models come to mind to address any of the dilemmas outlined above?

Healthcare and STEM Education – Siamese Twins in Reform and Innovation

The US has lost ground in recent years to other leading industrial nations in attracting new generations to science, technology, engineering, and math (STEM) careers. This has resulted in decreased enrollment rates in STEM college and university programs. Much of this trend is related to issues of global outsourcing of many of the related jobs in established industries, thus eliminating corresponding career incentives for high-school graduates. Industry and academia are in agreement that this educational trend is threatening the US economic and intellectual future and is one of the biggest challenges for current generations. Other reasons are related to an antiquated school system that has not changed significantly since the industrial revolution, and is, therefore, still favoring universal education over academic excellence on the PK-12 level.
National reform of our public school systems will take a long time, so one shorter-term solution is to specifically on women and minorities that are traditionally underrepresented in these careers. We need to find new approaches to attract minority groups to STEM programs, plain and simple.
One way we might do that is to embed STEM curriculum into currently desirable career fields. For instance, in a previous career at the University of Rhode Island, I wrapped traditional computer science education into the context of game design in order to attract more students. Another related initiative combined colleagues’ traditional STEM education with industrially relevant experiences and international exchange in order to emphasize the diversity and breadth of related career paths.
I will admit, however, that while this kind of approach can demonstrate isolated successes, it cannot change fundamental issues in career choice that are closely tied to gender differences: Although girls increasingly outperform boys in K-16 education, consequential female dominance does not seem to translate well into higher education or even STEM careers.
An extensive body of scientific research suggests that the apparent difference of career choice is in part related to gender differences in risk preferences, social preferences and competitive preferences. These differences have largely evolutionary roots, leading apparently – together with workplace discrimination and social acceptance pressure – to women’s ‘attraction’ to jobs with lower mean, lower-variance salaries. This relationship between evolved gender differences and occupational segregation might be hard to influence, so a bigger benefit may come from channeling these differences into new opportunities:

“The tendency of men to predominate in fields imposing high quantitative demands, high physical risk, and low social demands, and the tendency of women to be drawn to less quantitatively demanding fields, safer jobs, and jobs with a higher social content are, at least in part, artifacts of an evolutionary history that has left the human species with a sexually dimorphic mind. These differences are proximately mediated by sex hormones.”

What about healthcare reform?

It is widely accepted that healthcare reform will heavily rely on information and telecommunication technologies. Whether you are talking about electronic medical records or personal health records, telemedicine, telemonitoring or teleconsultation, online social communities of interest, remote caregiving, or Aging in Place, the trend from provider-centric healthcare to home- and individual-centered health and wellness is on the horizon.

This new found demand is creating an unprecedented need for scientific, technological, and engineering innovations. The corresponding career paths have the potential to combine both the job recognition and safety with the social content and rewards according to studies sought after by many women.
Could a potential to channel gender differences into new opportunities lie somewhere within healthcare reform? Early signs point to yes. Women are at the forefront of many of the emerging multidisciplinary research fields underlying the aforementioned healthcare IT R&D opportunities. These research fields combine aspects of Computer Science and Computer Engineering with Psychology, Social Sciences, Anthropology, Medicine and Communication and include Human-Computer Interaction, Affective Computing, Privacy Engineering, Health Communications (incl. Games for Health), Assistive Robots, and Online Social Networking, to mention a few. These women are the role models for new generations of women in STEM careers.
However, the emergence of such role models and the mere existence of the described opportunities for reform in health, wellness, and STEM education are not enough to catalyze the rapid change that is required. STEM education reformers are struggling to understand how to attract women and minorities to traditional STEM higher-education programs. There is also a struggle in determining how to develop frameworks for providing stronger workplace support for these underrepresented groups in STEM careers.
Where are the STEM curricula and initiatives specifically addressing the opportunities promised by home-centered healthcare and personalized health and wellness?
Where are the interdisciplinary centers communicating these opportunities to today’s high-school graduates?
And where are the public-private partnerships that can provide political decision makers with the implementation frameworks to link healthcare reform to STEM education reform?
The answers to these questions will not come from tweaking standardized testing or from providing in schools an additional hour of health and wellness per week … So where will they come from?

Living Media – A Forum to discuss how Emerging Technology is shaping the way we live, love, and thrive

This blog aims at providing a discussion forum on the topics of the media and communication technologies that are changing and will continue to change the way we live our lives, pursue our goals, and strive for self-efficacy in the presence of a supporting human network.

In doing so, it hopes at bringing together perspectives from leading scholars, practitioners, evangelists, and curious followers in the field who discuss the state of the art and the visions beyond the hype.

By placing these media into a common context this blog addresses the need of the general audience to understand the opportunities and challenges of the technology they are confronted with as well as the need of the research communities involved in the development and application of this technology to connect and develop a coherent vision. It is not our intent to prefabricate the vision but rather to inspire stakeholders and communities of interest to engage around a common application to the benefit of all – life. The technologies in mind encompass many of the important emerging media that are expected to change the way we live our lives and communicate in a world where technology is ubiquitous.

Since the industrial revolution, the increased mobility of populations has lead to a disruption of community and the dispersion of family as people pursue their goals in life. The increased need for supporting distant communication to the people dear to us and important in our lives, such as family members, friends, co-workers, physicians, and alike has driven the development of new technology for building and maintaining social relationships. While early we were only able to communicate words and images from a mainly separate context, new media have been developed that promise to take human-human communication to a new level allowing us to develop emotional bonds, form affections, and foster trust.

These media and related technologies support unique aspects of human intercommunication, such as creating virtual communities of interest (online social networks), jointly exploring and experiencing new environments (virtual worlds), referring to the current context in a conversation (location-based services), communication anyplace, anytime (mobile media), persuasion (video games and other persuasive media) and personalization.
